PMMA vs. HA for girth enhancement: the honest comparison

Most clinics answer this question with whichever product they happen to stock. We offer both, so we can afford to tell you the truth: neither is "better." They're different tools for different men, and choosing wrong is expensive in both directions.

The one-sentence versions

HA (hyaluronic acid) is a reversible gel filler — the same family of material used across aesthetic medicine for decades. It settles quickly, feels natural, and can be dissolved in a single visit if you change your mind.

PMMA is a collagen-stimulating filler. The microspheres themselves stay, and your body builds its own tissue around them. The result is effectively permanent — which is exactly its appeal and exactly its risk.

Where HA wins

Reversibility is not a small feature. For a first-time patient, the ability to undo the decision is worth more than most men realize until they need it. HA lets you live with a result before committing to it forever. It also integrates softly — in our experience the most forgiving material for men worried about how a result will feel to a partner — and it's done in a single session.

The trade: HA metabolizes. Expect maintenance over time, and expect the per-year economics to look worse than PMMA's if you maintain indefinitely.

Where PMMA wins

Permanence and economics. Staged correctly, PMMA is a one-time project — over a decade, meaningfully more economical than maintaining HA. And for men seeking a more substantial change, PMMA's collagen response builds real tissue rather than holding a gel in place.

The trade — and we say this plainly: permanence cuts both ways. A poorly placed permanent filler is a poorly placed permanent filler. This is why PMMA belongs exclusively in the hands of physicians who stage conservatively, and why a meaningful share of our revision practice is correcting PMMA placed aggressively elsewhere. It's also worth knowing that PMMA products such as Bellafill® are used off-label for this purpose — a fact any honest practice will volunteer before you ask.

The honest decision framework

First enhancement, any hesitation at all: HA — rent before you buy. Certain, informed, and thinking in decades: PMMA, staged over multiple conservative sessions. Bad experience elsewhere: revision first — we assess and correct before adding anything. Some men do both in sequence: HA as the trial year, PMMA as the long-term commitment once they know exactly what they want.

That your anatomy favors one option. That your expectations need adjusting before either. Occasionally, that neither is right for you. If your goals and the science don't align, we'll say so — the consultation fee is credited toward treatment, but the advice is honest either way.
